Who can access Support Coordination?
If you’re living with a disability, mental health issues, or you simply need extra help due to age, illness, or injury you can access our Support Coordination service.
Can I access a Support Coordination consultation remotely?
Yes, of course. Consultations can be held over the phone, via FaceTime, through email, or in person.
