Transitional Support Program

This program provides case management support to families experiencing homelessness or at risk of homelessness.

Support workers provide holistic case management to address long term housing goals, build capacity to reduce the cycle of homelessness and develop community links.

This program also provides support to families in transitional accommodation.

medium term support to access and maintain appropriate, longer term housing
development of holistic family support plans to help you identify and work towards your goals
practical support to develop independent living skills
assistance to access other appropriate services that you identify including financial and material aid, health services, family support and other identified specialist services.
assistance to connect with education, training and employment

Who it's for

Families with dependent children that are homeless with links to Casey and Cardinia.

Referrals via homelessness entry points only.

Growing up was long hard struggle for Jessica, who describes her mother as being verbally abusive towards her. An already troubled relationship with her parents became even more strained when mum and dad separated; leading Jessica to pack her bags and left home at 18.
