This program provides case management support to families experiencing homelessness or at risk of homelessness.
Support workers provide holistic case management to address long term housing goals, build capacity to reduce the cycle of homelessness and develop community links.
This program also provides support to families in transitional accommodation.
Families with dependent children that are homeless with links to Casey and Cardinia.
Referrals via homelessness entry points only.

(Through Kids on Track) I learnt more about how drugs and alcohol affect our bodies. I don't want to end up the same way as Dad so I can now care for my body and make better decisions than what he did.
